About Us
Established in 2019, Triveni Care was founded by a dedicated group of professionals driven by a genuine commitment to supporting vulnerable adults with mental health diagnoses, complex behaviours, and learning disabilities. Since opening our doors in October 2019, we have developed a diverse range of supported living services, becoming a trusted partner for our stakeholders and earning a strong referral base built on word-of-mouth recommendations—an achievement we take great pride in.
We are committed to delivering high-quality, person-centred care tailored to the unique needs of each individual. Our approach focuses on fostering independence and empowerment through active involvement in planning and decision-making, starting from the assessment stage. Supported living is a service designed to help individuals with various support needs live as
independently as possible in their own homes. It combines suitable accommodation with tailored
support to help individuals manage daily tasks, develop life skills, and engage in their communities.
Our services are open to adults with mental health needs, autism and learning disability or other
complex care requirements who would benefit from support while living independently.
We work closely with each individual, their families, and relevant professionals to create a
personalised support plan. These plans are designed around the person’s goals, needs, preferences,
and abilities, ensuring the right level of assistance is provided.
We offer a range of shared houses – All homes are safe, comfortable, and designed to promote
independence.
Yes, we provide 24-hour support where needed, including sleep-in and waking-night staff, to ensure
individuals feel safe and supported at all times.
Absolutely. We encourage family and friends to visit and remain actively involved in the lives of the
people we support, as meaningful relationships are vital to wellbeing.
Our support is focused on building confidence and life skills-such as cooking, budgeting, and travel
training-so individuals can gain greater independence over time while knowing support is always
available if needed.
We offer a variety of activities tailored to individual interests, including day trips, sports, creative
workshops – Art Psychotherapy and Drama Psychotherapy, social clubs, and community events to
encourage engagement, wellbeing, and enjoyment.
